You must be an existing client to recieve this service

To have an ID card sent to the contact person designated by your employer please complete the form below. ID Card request email must include group name, group number, and contract number. Your new BCBSM ID Card should arrive in 5-7 business days.  Please note that the contract-holder’s name is shown on all ID cards.

Members of the Michigan Osteopathic Association (MOA) can request their ID Cards here through Association Benefits.  ID Cards can also be requested directly through the insurance administrator at MOA by clicking here.
